- Replace [COMMAND] tag protocol with proper bash tool definition
- Add add_assistant_message/add_tool_result for correct tool call history
- Default model changed to cohere/north-mini-code:free
- System prompt: add Plan First step, simplify file editing, clarification gate, git awareness
